No Show or Late Cancellation: 50% of the appointment fee for the 1st occurrence. Subsequent occurrences will be charged the Full Fee.
Paperwork: Paperwork completion, letters, or other similar items will be billed at the 25 minute appointment rate based on time for completion.
2 business days are required for cancellation and rescheduling. This means that for a Tuesday appointment, we must receive notice by the previous Friday to avoid the late cancel/no show fee.

I am out of network with all commercial insurances, including Medicaid and Medicare. A valid credit card on file is required for all patients. We can provide a superbill which you can submit to your insurance for out of network benefits. We accept all major credit cards, HSA’s, or debit cards for payment. If you are planning to seek reimbursement, I strongly encourage you to contact your insurance carrier to find out about your out-of-network benefits for diagnostic codes 99205, 99213, 99214, and 90836 so that you can anticipate your out of pocket expenses prior to scheduling an appointment. Payment is due at the time of service.
Remaining out of network allows me to have a smaller patient panel, and this supports the capacity to have longer appointments than those typical of an insurance based practice.
Thank you for considering me to be your doctor. All potential patients will have a screening call before an appointment is offered. I will begin screening new patients again in April 2026.
To become a new patient, first please fill out the New Patient Appointment Request Form. A new patient screening call will then be scheduled by the patient care coordinator. This call does does not establish a physician-patient relationship. The physician-patient relationship begins after our first in-person or video consultation appointment.

I offer both in-person and virtual appointments. The 1st visit is required to be in person/in office. Subsequent appointments can be virtual, and annual in-person visits are recommended. All patients should be located within the state of Texas or North Carolina for appointments. State licensing rules require that the patient be located in a state where I have an active medical license.
Prior to the covid-19 pandemic, the law known as the Ryan Haight Act required at least one in person appointment prior to prescribing any controlled substance. This law was suspended during the pandemic, and in-person appointments are still not currently required by law. However, the law may go back into effect in the future, and this would require an in-person appointment annually for the maintenance of a controlled substance prescription.
Virtual visits are always recommended for anyone experiencing flu-like/upper respiratory symptoms, fever within the past 24 hours, who are unvaccinated against flu and covid, who have had recent close contact with a known covid positive individual, or who are otherwise feeling unwell.
Hepa filters are present in Dr. Maxwell's office and in the waiting room.
The short answer is, "it depends." Patients need to be seen a minimum of 3 times per year when medications are stable and well-established. Appointment frequency is determined collaboratively. Most patients require more frequent appointments, particularly for the first 6-12 months of treatment.
Under Federal law, you have the right to a "Good Faith Estimate" explaining how much your care will cost during a one year period.
During appointment scheduling, we will review the fee schedule as well as likely anticipated appointment frequency.

Can you complete my forms for disability, accommodations requests, or other types of letters?
I do not complete requests for disability evaluations within the 1st appointment. Paperwork and forms for disability are generally only completed once care has been established for a minimum of 6 months. Paperwork and form completion will be billed at the 25- minute rate. Sometimes an appointment is needed specifically for form completion. I do not routinely complete letters for emotional support animals. Family Medical Leave Act forms must be discussed in advance of taking any leave and any form completion.
